How to get a birth certificate in Mumbai- September 2015

Note:-The hospital will forward a supporting document to the ward office directly, confirming child's birth. This might take atleast a week. Now, check which ward does your maternity hospital belongs.
 
For my daughter's birth-certificate, I visited the  K/West ward, Municipal office in Andheri. Here's how I got the birth certificate in awesomely shocking 15-20 minutes. 
  1. On the first floor of K/West ward, Municipal office in Andheri, is the health department. Get a birth certificate form from here. 
  2. Fill the form carefully. One form can fetch a maximum of 5 original copies of birth certificate.
  3. Attach a copy of the applicant's photo id proof.
  4. Submit the form and copy of photo id to the Common Facility Centre (CFC) on the ground floor at one of the counters. 
  5. The official at the counter will show the computer screen to confirm all the information entered. Please read twice as the name change procedure may take more time! 
  6. One original copy will cost around Rs. 30/-
  7. The receipt of submission and payment,  will immediately fetch, original birth certificate at one of the counters. (In my case it was the extreme left one)
  8. Again visit the first floor's health department. The official there will help you get a stamped and signed original birth certificate
  9. It's done in 15-20 minutes! I was amazed at the efficiency of the department
